Medical Cards
A Medical Card is a must to have and carry in your wallet or purse. Just in case you are in an accident, someone will have access to your medical information.
The card holds several important information of family and friend who need to be informed if you are in an accident or hospital and can also look after and feed your pet.
You can personalise this card with all your medical, allergies and emergency contact details.
Printed both sides on white plastic credit card material 86 x 54mm.
Printed in high quality on hard PVC plastic, this card will fit perfectly in your wallet or purse and is scratch and fade resistant.
This is not an official card but can be used as a convenient way of presenting your important medical information.
The card can contain any or all of the following information: photo, name for the card, date of birth, hospital id no, national health service number, your address, any medical conditions, list of medications taken, emergency contact name and phone number(s)
Please be aware that the lettering can be quite small and we will always ensure that it is readable and contact you prior to printing if there are any issues.
Please ensure that all medications and medical conditions are spelt correctly as we cannot be held responsible for any errors in this regard. It may be necessary to abbreviate some medications or medical conditions in order to fit all the information on to the card.
These cards do not constitute proof of identity and should only be used in circumstances where you need to provide the emergency medical information to a health care professional.
We do not store your information or details. Data is only held for the production of the card(s) and then immediately removed from our systems. All information is handled securely and with full discretion by our CRB/DBS checked staff.
