Chester Step Test (Aerobic Fitness Test)
The Chester Step Test is a standardised, progressive submaximal aerobic fitness assessment used to estimate your aerobic capacity (VO₂max). It is commonly used in occupational health to support fitness-for-work decisions where roles involve physical demands, safety-critical tasks, or (where applicable) the use of respiratory protective equipment (RPE).
At Your Excellent Health Service (YEHS), the test is delivered by experienced clinicians in a controlled clinical environment, with clear reporting for individuals and employers (where consent is provided).
The Chester Step Test can be used by Emergency Response Teams (ERT) in the oil and gas sector or by firefighters or seafarers as maintaining a high level of fitness is essential. Certain job roles response to acute or life-threatening situations, making physical readiness critical. Therefore, the aerobic fitness testing is an occasional additional requirement.
What Is the Chester Step Test?
During the test, you step up and down on a fixed-height platform in time with a paced audio track (metronome). The pace increases in stages (typically every 2 minutes). We monitor your heart rate response, how hard the exercise feels, and any symptoms you report.
Because it is submaximal, you are not expected to exercise to exhaustion. The assessment usually aims to reach a safe endpoint—often around 80% of predicted maximum heart rate—or earlier if clinically indicated.
Who is the test for?
The Chester Step Test may be suitable if you:
- Need an occupational health fitness assessment for a physically demanding role
- required as an additional test for yoru ENG1 (seafarers) medical
- Work in a safety-critical environment where aerobic fitness is important
- Are undergoing assessment for offshore / marine / emergency response duties (where required by your role or employer)
- Are returning to work after illness or injury and need a structured fitness check
- Want a baseline measure of fitness to guide a safe improvement plan
If you have symptoms such as chest pain, unexplained breathlessness, dizziness/fainting episodes, or known heart conditions, we may recommend a clinical review first to ensure the most appropriate assessment is selected.
Why Is It Important?
Aerobic fitness is a key part of functional capacity for roles that involve sustained activity, manual handling, emergency response, or working under time pressure. The Chester Step Test provides an objective estimate of aerobic fitness which can help:
- Support fitness-for-work decisions
- Identify whether further assessment or adjustments may be needed
- Provide a baseline for safe, measurable improvement over time
For some employers and roles, repeat testing may be requested (for example annually). Requirements vary depending on your job role, employer policies, and applicable industry standards.
What happens during the appointment?
A typical appointment includes:
- A brief screening and explanation of the test
- Stepping in time with the paced audio track as the intensity increases
- Monitoring of heart rate throughout the test
- Recording of perceived exertion (how hard the exercise feels) and any symptoms
- Additional checks (e.g., blood pressure) where clinically indicated
You can stop at any time. We will also stop the test if safety thresholds are reached or if you develop symptoms.
Appointment time: please allow around 30 minutes (this may be longer if combined with another medical assessment, such as an offshore medical).
What you receive
After your assessment, you will receive a clear outcome summary. Where required (and with your consent), we can provide an employer-facing report outlining:
- Estimated aerobic capacity and functional interpretation
- Any relevant limitations identified during testing
- Practical recommendations for work adjustments (if needed)
- Suggested review timeframe and any follow-up recommendations
