Asbestos Medical Surveillance (HSE Appointed Doctor)
If your employees carry out licensed asbestos work — and in some cases certain non-licensed work — employers must ensure staff are under adequate medical surveillance by a relevant doctor. YEHS provides compliant assessments and certification.
Dr. Ade Buluro is an HSE Appointed Doctor providing statutory medical surveillance for workers exposed to asbestos and lead. YEHS delivers compliant assessments, clear certification, and employer-ready documentation.

Dr Charlie or Dr. Ade can be appointed as your personal company medical officer to help reduce rates of absenteeism. They will be able to give advice on when a referral to occupational health is required and ways to monitor, prevent and manage sickness absence. In many cases this will assist the employee in a faster and safer return to work by recommending adjustments or modifying duties. YEHS offers face-to-face, telephone or video consultations when making an assessment.
What this service provides

- advise on whether to continue working with asbestos
- Certification following examination (for employer and employee)
- Licensed Asbestos Work:
- We will issue an original certificate of medical examination (FODMS72A) to both the employer and employee (or self-employed person).
- The employer must keep the certificate or a copy of that certificate for at least four years from the date of issue.
- The FODMS72A Certificate does not comment on fitness/unfitness for work with asbestos as this is not required by CAR.
- If a fitness for work assessment is requested, this is done separately and in addition to the asbestos medical. The outcome of this risk based fitness assessment is not documented in the FODMS72A.

- We will issue an original certificate of medical examination to both the employer and employee (or self-employed person).

How often is surveillance required?
Medical surveillance should consist of initial and periodic medical examinations. Surveillance intervals depend on the work category:
- Licensed Asbestos Work: The first medical examination must be conducted no more than two years before beginning exposure to asbestos. Periodic Medical examinations must be conducted at intervals of no more than two years while exposure continues. The doctor may use his discretion to shorten this interval where appropriate.
- Non – Licensed Asbestos Work: The first medical examination must be conducted no more than three years before beginning exposure to asbestos. Periodic Medical examinations must be conducted at intervals of no more than three years while exposure continues. The doctor may use his discretion to shorten this interval where appropriate.
What the process involves
This process depends on the work category.
- Licensed Asbestos Work:
- completion of a medical surveillance form (FODMS75) (This is not required as part of medical surveillance for firefighters) (Click here to fill form)
- taking a medical and occupational history, with particular reference to respiratory conditions, smoking habits and previous exposure to asbestos, other fibres or dusts
- completion of the respiratory symptom questionnaire form (MSLW1) (Click here to fill form)
- FORMS: We will send you a link via email after booking to fill the relevant forms
- competent clinical examination, with emphasis on the respiratory system and the presence of basal crackles (documented on the health surveillance record form FODMS100); and
- Spirometry (Lung Function Test), including FEV1 and FVC.
- A Chest X-ray is not routinely required. It is only requested by the HSE Appointed doctor on individual clinical grounds with a clear explained reason for this. If a chest X-ray is required, he may use a recent chest X-ray available from other sources.
- Non – Licensed Asbestos Work:

- competent clinical examination, with emphasis on the respiratory system and the presence of basal crackles; and

What we need from you
- Work category (licensed vs non-licensed; brief description of tasks)
- Any previous asbestos medical certificates/dates
- Preferred appointment location and timeframe
What Employers Recieve
- Certificate confirming the employee has been examined and the date of examination
- Clear next steps if any concerns are identified
- Practical guidance prompts for employer risk review where appropriate
YOUR DATA: Retain of Health Records
- A Health record for each employee includes personal details of the worker, a record of the types of work carried out involving asbestos and the dates of medical examinations
- It must be kept for at least 40 years from the date of the last entry.
YOUR DATA: Retain of Clinical Records
- A Clinical record for each employee includes completed respiratory symptom questionnaire, details of examination findings and any tests conducted as part of the medical examination.
YOUR DATA: Asbestos Worker Database
Medical Surveillance forms (FODMS75) with data on the number of medical examinations are periodically reported to HSE and added to the Asbestos Worker Database.
YOUR DATA: Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR)
Mesothelioma or lung cancer in workers occupationally exposed to asbestos are reportable diseases under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR). Employers and self-employed people have a duty to report these diseases when informed in writing of the specific diagnosis by a registered medical practitioner.
