Dr Charlie or Dr. Ade can be appointed as your personal company medical officer to help reduce rates of absenteeism. They will be able to give advice on when a referral to occupational health is required and ways to monitor, prevent and manage sickness absence. In many cases this will assist the employee in a faster and safer return to work by recommending adjustments or modifying duties. YEHS offers face-to-face, telephone or video consultations when making an assessment.

What to expect 

  1. Book your appointment
  2. Bring your D4 form and required documents (see checklist below)
  3. We complete the medical examination and the relevant D4 sections
  4. You submit the completed D4 as part of your DVLA application/renewal process (as instructed by DVLA)

  • Your D4 form (unless you have arranged for us to provide/print it)
  • Photo ID (for identity confirmation)
  • Glasses/contact lenses if you use them for driving
  • A list of current medications (or repeat prescription printout)
  • Relevant supporting letters/summaries for any ongoing conditions (e.g., diabetes, sleep apnoea, cardiac, neurological, psychiatric, eye conditions)
